Your care plan is the roadmap for your visits. It describes what type of care you'll receive, how often, and who will provide it. A care plan includes the care recipient's name, how often visits happen (daily, twice a week, etc.), the time of day, and which caregiver is assigned. It also lists the specific tasks your caregiver will help with during each visit—things like bathing, meal prep, medications, exercises, or companionship. The plan shows the type of care: assistance with daily living, companionship, skilled nursing support, or another service your agency provides. This tells you the scope of what each visit covers. Tap the Plans tab at the bottom of the app. You'll see all your active care plans listed with the caregiver name, visit frequency, and status. Tap any plan to open it and see the full details: the care tasks your caregiver is responsible for, which caregiver is assigned, hours per week, and all your upcoming visits under that plan. If you want to change the visit time, add or remove tasks, adjust how often visits happen, or make any other change, message your agency through the Messages tab. Describe what should change and why. Your agency will review your request and let you know what they can do.
